It is possible that some services or "Stacked Treatments" can be completed during the same visit.
Serivces will only be performed if safe to do so, per staff discretion, and appropriate healing time has elapsed between treatments.
We will work with you to create a plan in order for you to get the most out of your membership.
VIP Membership $100 + GST/mo
The membership is valid for a minimum of a 12 month term.
Credit Card Authorization and Payment Terms
- Your credit card information will be kept on file and will be automatically charged $100 + gst on the first day of each month.
- Membership payments are non-refundable.
- If the credit card payment is declined, the clinic will contact you to provide an alternative form of payment.
- If a new payment method is not provided within 30 days of the declined payment, the membership will be considered terminated, and you will be charged the full price for any services received at discounted membership rates. Immediate payment of these charges will be required upon termination.
Membership Term and Cancellation
- This membership is non-transferable
- This membership is valid for a minimum of a 12 month term.
- No refunds will be issued for membership payments or services provided.
- If you choose not to renew after the initial 12-month term, you must provide written notice at least 30 days prior to the renewal date to avoid further charges.
Promotions and Discounts
- You will always receive the lowest available rate. You will receive any promotional discounts offered if applicable.
Termination and Charges for Discounts Received
- If the membership is terminated due to non-payment, the Member will be billed for the difference between the discounted rates received and the full price of services used during the membership period.
- Payment will be due immediately upon receipt of this bill.
